How do I become a patient?

What are the different services you offer?

Functional Medicine Consultations: We are not your typical physicians. We work together to determine the best way to help you reach your goals to health and wellness. Every patient starts with a free consultation to discuss your current concerns. If it’s a good fit we will schedule your initial appointment and exam as well as run functional labs that will give us more information about your health. You can schedule this consultation here.

Chiropractic: We specialize in treating and fixing movement patterns because prevention is vital for making sure both acute and chronic injuries heal and don’t come back. You can schedule a new patient appointment here.

Massage: Utilizing the best possible massage techniques to promote better function and healing as well as encourage relaxation for each patient. You can schedule an appointment here.

Do you accept insurance?
We do not accept insurance or medicare. Some insurance carriers may cover medical services and laboratory tests performed by our clinicians. We can provide a detailed receipt for services performed for you to submit to your insurance carrier directly.
Do I need to travel to a Northwest Functional Medicine clinic for an appointment?
Yes, your first appointment with a Functional Medicine physician must be done in-person.

Consultations can be performed over the phone; however, all new patient exams and appointments must be performed at one of our clinics.

Follow Up Appointments for Functional Medicine can be done over zoom or phone conference. Give us a call to learn more if you’re traveling from out of state.

Can the physicians at Northwest Functional Medicine be my primary care physician?

We require that you have your own primary care physician. Our physicians are trained as primary care physicians, but we work with chronic illnesses and do not provide acute care services. We provide preventive, nutritional and chiropractic care to help you address the root cause(s) of chronic health problems.

What is your cancellation policy?
For all appointments cancelled within 48 hours of the appointment date, there will be a 50% cancellation fee due immediately upon cancellation. This fee can be applied to any appointment that is rescheduled within 6 months from the original appointment date.
